    For Salaried Employees/ Commissioned or Variable Income Earners
    -Latest Computerised Payslip OR
    -Latest Bank Statement
    -Latest 12 months CPF Contribution History Statement

    For Self-Employed -Latest Bank Statement
    -Latest Income Tax Notice of Assessment IR8A
    Singapore Permanent Residence (Singapore PR) Identity Card (Blue IC) and Malaysia IC - Applicant to sign on the copy of the Blue IC and Malaysia IC (Malaysia Citizen) OR Identity Card (Blue IC) and Passport - Applicant to sign on the copy of the Blue IC and Passport (Non-Malaysian Citizen) Work Permit or Employment Pass (If Applicable)

    For Salaried Employees/ Commissioned or Variable Income Earners

      1. Corporate Account Opening Form
      2. Risk Disclosure Statement
      3. CDP Form - Application for Opening of Corporate Securities Account
      4. CDP Form - Account Linkage / Revocation
      5. Director's Resolution For Opening of Corporate Securities Trading Account with DMG & Partners Securities Pte Ltd
      6. Letter of Authorisation to collection CDP account number
      7. Application for DBS Interbank GIRO
      8. Application for UOB Interbank GIRO


    2. Supporting documents to be enclosed with your application:

      - 2 copies of Certified True Copy of the "Certificate of Incorporation"
      - 2 sets of Certified True Copy of the "ACRA" and dated not more than 2 weeks from application submission date
      - Certified True Copy of "Memorandum & Articles of Association"
      - Copy of NRIC/ Passport for person authorised to operate the account
      - Copy of Bank Statement (not more than 3 months from the date of CDP Account Opening Application - For SGD only)
